The Field Care Manager Nurse 2 assesses and evaluates member's needs. We do this to achieve and maintain an wellness state. The nurse guides members/families toward resources and facilitates interaction with them. These resources are appropriate for the care and wellbeing of members. The Field Care Manager Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Field Care Manager Nurse 2 employs a variety of strategies, and techniques to manage a member's physical, environmental, and psycho-social health issues. Resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by overseeing patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas. Follow established guidelines/procedures.

Required Qualifications
- Must reside in the state of Kentucky or in a county that is contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia.
- Ability to travel to member's residence within 30 to 40 miles in the state of Kentucky.
- An active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
- 1+ years of maternal child nursing experience such as labor and delivery, neonatal ICU, postpartum/nursery.
- Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
- Knowledge of community health and social service agencies and additional community resources.
Preferred Qualifications
- BSN
- 3+ years of in-home assessment and care coordination experience.
- Experience with health promotion, coaching and wellness.
- Previous managed care or Medicaid health plan experience.
- Certification in Case Management (CCM).
- Motivational Interviewing Certification or knowledge.
Additional Information
- Schedule/Time Zone: Monday through Friday, 8:00 AM – 5:00 PM Easter Time Zone with flexibility to work overtime as per business needs.

- Work Style: Hybrid (90% Remote, 10% Field)
- Travel Requirements: 10% travel where employees perform their core duties at non-company locations, such as providing services at business partner facilities or prospects' and members' homes. Travel also includes attending face to face and collaborative meetings at Humana’s Louisville, KY location.
Work-at-Home (WAH) Internet Statement

TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
